Application Process
Please enter the necessary information on the form appeared on the screen. Upon finishing please print out the form and fax it. Once the fax is sent then your application is completed.
After we had recieved your fax you will receive a confirmation e-mail. If you do not receive an e-mail after 2 days please call us at 03-3440-3373 or E-mailqinfo@iyfnet-jp.orgr to Ms. Murakami.
We accept Visa/Master/UC/JCB. (No installment payments are accepted)
Cancellation
If you would like to cancel your donation, please contact us by e-mail or by phone. There is a possibility that the credit card company has already completed your transaction if you contact us after 3 days. If so it may take up to 2 months to receive your credit.
